When scheduling a procedure with our office, we will need the following information:

• Name

• Address

• Telephone number (s)

• Date of birth

• Insurance information

• Carrier name

• Identification number or claim number

• Date of injury (if no fault or workers compensation claim) (we accept most insurances)

• Ordering physicians name

• Procedure being ordered

Upon arrival at our office for the procedure, you will need to bring:

• Insurance card (s)

• Actual script or physicians order

• Copayment (cash, check or major credit card are acceptable)

• Driver, if having sedation (unless you are using our transportation)
